Care Coordination for the COVID-19 Crisis

Many people, particularly seniors, are isolated and facing unique challenges during this Pandemic. Their daily needs must be met, but the crisis has separated them from family, caregivers and services.

They may be coping with anxiety, loneliness and fear, whether in the home, hospital or in a senior facility.
